Fees & Payment Procedures:
We appreciate and encourage payment at the time of your visit. For your convenience, MasterCard, Visa, Discover
and American Express are accepted.
If you have questions regarding our fees,
please feel free to speak with our office manager.
Insurance:
Our medical services are charged directly to the
patient, not the insurance company. Therefore, our patients are responsible
for payment of medical services.
As you leave the office, you will receive a computer print out showing an
itemized account of services rendered. This computer print out gives codes for the services rendered along with diagnostic
codes. It should be used, along with your insurance form, for reimbursements
from your insurance company.
For our patients who have Blue Cross/Blue Shield of Michigan: We submit your claims directly
by computer as a courtesy. You will receive any reimbursement at home.
For those patients who have
Aetna, Blue Cross Blue Shield PPO, Cigna, Medicare
and Connect Care:
Payment of
the co-pay is due at the time of service.
